Terminal Knee Extension

Terminal knee extension strengthens the last degrees of knee straightening—key for fixing extension lag and gait after ACL reconstruction.

Terminal knee extension is end-range knee straightening against resistance—often with a band behind the knee while seated or standing—to strengthen the quads in the last degrees of extension. It targets extension lag common after ACL reconstruction and supports normal gait.

Keep pain low and avoid hyperextension forcing. Progress band tension as control improves.

Common questions

  • Does terminal knee extension stress the ACL graft?

When prescribed in early rehab, it is typically low shear at controlled angles. Follow your PT’s stage—do not add heavy band tension before clearance.
